To ensure a safe and healthy start to the school year, I am reaching out to families of students who require health plans or medication administration during school hours.
1. Schedule Your Consultation Please use the link below to book a 30-minute window to meet with me between August 26th and September 11th. During this time, we will finalize your student’s health plan and check in any necessary medications. If your student will be taking medications at school please schedule your appointment for in person.
2. Important Medication Safety Rules When you come for your appointment, please remember:

Adults only: Medication must be hand-delivered by an adult. For safety reasons, please do not send medication in your student’s backpack.

Original Packaging: Prescription meds must have the pharmacy label. Over-the-counter meds must be in a brand-new, unopened container.

Paperwork: Please bring a signed 👉 Medication Authorization Form for each medication.
